ADMISSIONS

Diamante Montessori School accepts children from the ages of 24 months to 5 years regardless of race, color, sex, and national or ethnic origin. Enrollment will take place after a tour of the school, meeting with the director, and a play visit.

The following forms are required for enrollment to our school:

• Admission Agreement

• Notification of Parent’s Rights

• Personal Rights Form

• Consent for Emergency Medical Treatment

• Identification and Emergency Information

• Child’s Pre-admission Health History-Parent’s Report

• Physician’s Report

• Immunization Card (Shot Records)
